Caregiver requirements
Minimum Requirements for our Support staffs and Instructors Per Title 17:

Be 18 years of age or older
-
Have the ability to dedicate the necessary time on site and involved with program activities to successfully perform duties of job and hours agreed to with the program.

Ability to converse in English, as demonstrated during oral interview
-
Must be in adequate health to perform the position as demonstrated by a health Screen & TB clearance verifying ability to perform duties of job within 7 days of hire or no more than 6 months prior to hire.

Fingerprint/criminal status clearance from the Dept. of Justice and Federal Bureau of Investigation prior to on-site employment.
-
Possess a high school diploma or GED certificate.

Sign an affidavit that applicant has never been convicted of a crime other than a minor traffic violation
Caregiver responsabilities
All Support Staff and Instructors must:
-
​Have the ability to implement and comply with job requirements and duties of a direct support professional and all necessary regulations/requirements.

Assist and maintain consumer records and document consumer status, as needed.

Communicate and cooperate with agency services, consumer, and consumers’ authorized representative.

Comply with program policies and procedures and with all legal requirements, i.e. Title 17 for SLS and ILS program.
-
Comply with consumer’s rights, and mandated reporting requirements.
-
Assist consumers in all needed personal grooming such as bathing, dressing, medical and dental needs.

Maintain the individual homes and surroundings in clean, attractive, and safe manner.
8. Report any equipment failure, hazards, or unsafe living situations immediately to Program Director.
9. Notify Program Director/designee of any significant change in consumer status and well-being, of any grievance, neighborhood complaint, and unusual occurrence.
10. Provide incidental health care, i.e., assistance with administration of medication, sick day assistance, First Aid, and transportation to medical appointment.
11. Assist with any medical urgencies and/or emergencies
12. Prepare, serve, and store meals and snacks per stated menu.
13. Provide transportation and/or accompany and supervision during medical appointments, community, and recreational outings.
14. Report any unusual occurrences or suspected abuse within 24 hours to SARC, and Program Director.
15. Document any unusual occurrence or suspected abuse and comply with mandated reporting requirements.
16. Train or Support consumers per Individual Program Plan.
17. Maintain appropriate communication with others supporting consumer, e.g., family, Circle of support and SARC.
18. Attend person centered and planning meetings, as needed.
